Job Summary
Assist patients with activities of daily living including bathing, dressing, grooming, and toileting. Provide mobility assistance, transfers, and repositioning as needed. Support meal preparation and light housekeeping related to patient care. Monitor and report changes in patient condition to the supervising nurse or care coordinator. Offer companionship and emotional support while maintaining a safe, clean, and supportive home environment. Document care provided accurately and in a timely manner following individualized care plans and agency policies. This role is zoned in South/Southwest Indiana with flexible scheduling.
Required Qualifications
- Valid Indiana HHA or CNA certification
- Positive attitude and compassionate, patient-centered approach
- Reliable transportation and valid car insurance
- Physical examination within the last six (6) months
- Current TB skin test
- Ability to pass a background check
- Must pass a drug screen upon offer of employment
Desired Qualifications
- Prior home health experience
