Job Summary
Provide patient care related services and tasks as ordered by the physician in the plan of care. Maintain a valid state driver license, automobile liability insurance, and dependable transportation in good working condition. Complete training and competency evaluation in compliance with federal and state regulations, and hold current CPR certification. This role requires one year of direct client care experience in an institutional or home health setting and registration on the state nurse aide registry. Candidates must demonstrate basic technology skills or show an active willingness to learn them. Required Qualifications
- Must meet one of the following requirements: 1) have a minimum of one year full time experience in direct client care in an institutional setting (hospital or nursing facility); or 2) have one year experience within the last five years in direct client care in a home health or hospice setting.
- Must be registered and listed in good standing on the nurse aide registry in the state of employment.
- Must have basic demonstrated technology skills, including operation of a mobile device.
- Must possess a valid state driver license
- Must maintain automobile liability insurance as required by law
- Must maintain dependable transportation in good working condition
- Must be able to safely drive an automobile in all types of weather conditions
- Must complete training, competency evaluation in compliance with federal and state regulations
- Must possess CPR certification

- CONNECTICUT - Must hold a certificate as a certified nurses assistant
- DELAWARE - Must have completed one year in a department approved facility setting; or completed 75 hours of instruction training with practicum and supervision; or be student nurse and completed clinical practicum.
- FLORIDA - Must hold a home health aide certificate
- IDAHO - Must hold a certificate as a certified nurses assistant
- ILLINOIS - Must hold a certificate as a certified nurses assistant
- KANSAS - Must hold a home health aide certificate
- MARYLAND - Must hold a certificate as a certified nurses assistant
- MONTANA - Must hold a home health aide certificate
- NEVADA - Must hold a certificate as a certified nurses assistant
- OKLAHOMA - Must hold a certificate as a certified nurses assistant
- OREGON - Must hold a certificate as a certified nurses assistant
- TENNESSEE - Must hold a certificate as a certified nurses assistant
- TEXAS - Must hold a certificate as a certified nurses assistant
- UTAH - Must hold a certificate as a certified nurses assistant
