Job Summary
Home Health Aide (HHA) per diem/PRN providing direct patient care under RN supervision in patients' homes. Responsibilities include assisting with medications, monitoring vitals, maintaining hygiene and nutrition, mobility and transfers, feeding, catheter/ostomy care, infection control, and documenting care. Performs tasks per Plan of Care, communicates changes to supervisor, and supports in light household duties and transportation as needed. Requires completed HHA/CNA program within 24 months, current state certification, valid driver’s license and auto insurance, good standing on HHA Registry if applicable, and at least one year of recent HHA/CNA experience. Required Qualifications
- Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
- Must meet applicable state certification requirements
- A valid driver's license, auto insurance, and reliable transportation are required
- Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
- Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
