Job Summary
Provide direct patient care as a Home Health Aide under RN guidance, assisting with medications, vital signs, hygiene, feeding, mobility, transfers, and activities of daily living per the Plan of Care. Perform documentation of interactions and care, observe dietary intake, maintain infection control, and support light housekeeping and transportation tasks as needed. Adhere to TB screening requirements and state-specific certification practices; must hold a valid driver’s license and reliable transportation to travel to patients’ homes. Required Qualifications
- Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
- Must meet applicable state certification requirements
- A valid driver's license, auto insurance, and reliable transportation are required
- Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency.
- Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
