Home Health Aide- Private Duty

Monitor and record patient conditions, vital signs, and intake/output while assisting with bathing, dressing, grooming, and household tasks like laundry and meal preparation. Support the patient's Plan of Care by teaching families proper lifting techniques and collaborating with the multi-disciplinary care team to implement assessments. Interface with families to field concerns, educate them on treatment needs, and maintain accurate records in the EMR system. Perform on-call services as assigned and adhere to aseptic protocols and federal/state requirements. Maintain mandatory continuing education and hold active CPR certification. Requires a Home Health Aide certificate, one year of experience, valid driver's license, and strong communication skills.
