Job Summary
Assist patients with activities of daily living including bathing, dressing, grooming, and toileting. Provide mobility assistance, transfers, and repositioning as needed while preparing meals and performing light housekeeping. Monitor and report changes in patient condition to the supervising nurse or care coordinator, maintaining a safe, clean, and supportive home environment. Document care provided accurately and in a timely manner. This role focuses on promoting comfort, dignity, and independence for clients in New Haven. Required Qualifications
- Valid Indiana HHA or CNA certification
- Positive attitude and compassionate, patient-centered approach
- Reliable transportation and valid car insurance
- Physical examination within the last six (6) months
- Current TB skin test
- Ability to pass a background check
- Must pass a drug screen upon offer of employment

- Prior home health experience
