Job Summary
Provide in-home patient care as a Certified Home Health Aide (HHA) in Rochester, following the patient’s plan of care. Responsibilities include personal care, light housekeeping, meal preparation, errands, and collaboration with healthcare staff; maintain effective communication about patient status; accurately document care; adhere to HIPAA and all local/state/federal requirements. Must hold HHA certification NYSED and a valid New York driver’s license; perform duties with compassion, critical thinking, and the ability to work with minimal supervision; schedule is flexible.
