Job Summary
Home Health Aide provides in-home, one-on-one patient care following individualized care plans. Responsibilities include assisting with daily activities (transfers, baths, mobility, personal care), changing bed linens, laundry, light housekeeping, medication reminders, and engaging patients in activities to support mental and physical health. Maintains records of patient care and communicates observations to supervisor; builds relationships with patients through conversation or reading aloud. Strong verbal and written communication skills are required. Prior Home Health or Hospice experience is a plus. Requires graduation from an accredited Home Health Aide program, High School Diploma or GED, and one year of professional experience; driver’s license, background check, and drug screen are required. Required Qualifications
- Graduated from an accredited Home Health Aide program
- High School Diploma or GED
- One-year prior professional experience
- Driver’s license required
