Job Summary

The Home Health Aide provides essential in-home patient care under RN/LPN supervision, performing ADL assistance, monitoring patient condition, and reporting changes to supervising nurses to support patient comfort, safety, and independence. Responsibilities include assisting with bathing, grooming, dressing, toileting, feeding, mobility, ambulation, transfers, and positioning; providing companionship; administering over-the-counter medications as delegated; supporting treatment plans (including catheter care and dressing changes) within Illinois scope of practice; maintaining accurate documentation; reinforcing disease management and infection prevention education; and coordinating with the clinical team to ensure care continuity. Must uphold HIPAA, CMS/JCAHO/IDPH standards, infection control, safety protocols, and maintain professional communication with patients, families, and interdisciplinary staff. Requires high school diploma or GED, active Illinois CNA certification, 6–12 months of related experience preferred, reliable transportation, and ability to travel to patient homes. Knowledge of electronic documentation tools and basic office software may be required. Frequent home-based visits with travel to multiple residences; standard home health shifts and flexibility for urgent or weekend visits.

Required Qualifications

  • Education: High school diploma or GED required
  • Completion of state-approved CNA training program
  • Current Illinois CNA certification required
  • Experience: Preferably 6–12 months in home care, long-term care, or related patient care setting
  • Knowledge: Basic understanding of patient care techniques, infection control, safety protocols, and HIPAA regulations
  • Skills: Strong interpersonal, communication, and observational skills; compassion and patience in providing care
  • Technology: Ability to use electronic documentation tools and basic office software if required
  • Other Requirements: Valid driver’s license, reliable transportation, and ability to travel to patient homes
