Job Summary
Provide one-on-one in-home care to patients in Velarde, NM per physician-established care plan; duties include assisting with activities of daily living, light housekeeping, meal preparation, and monitoring vital signs; maintain documentation and communicate with patients’ family, doctors, and other care team members; role is a 1099 contract opportunity with a schedule of 35 hours per week (Mon-Sun, mid to late morning); collaborate with CNAs, HHAs, Case Managers, and physicians; CPR certification is required.
