Home Health Aide (HHA or CNA)

Provide one-on-one in-home care to patients using an established care plan; duties include activities of daily living assistance, personal care, light housekeeping, meal preparation, monitoring vitals, reminding medications, documenting services, and communicating with family, doctors, and prescribed individuals. Collaborates with CNAs, HHAs, case managers, and physicians to ensure patient needs are met under a physician-approved plan; 35 hours per week, Monday through Sunday with mid-to-late morning start, in Velarde, NM; 1099 contract position; background check required.
