Job Summary
Execute physician prescribed plans of care and compliant documentation in the system of record. Administer prescribed treatments and therapies while performing patient assessments and coordinating care. Provide various activities associated with daily living, including light housekeeping, preparing beverages, and general support with activities of daily living. Offer companion care and supervision during outdoor activities, maintaining safety standby while the patient performs yard work and uses a golf cart. Monitor vitals and communicate with family members, doctors, and other prescribed individuals regarding patient health. Work one-on-one with a male DOL patient in his home, encouraging independence while respecting his routines and preferences for medication management and privacy. Requires current CPR, First Aid, and 2-Step TB Test, with ability to lift up to 50 lbs. Schedule includes Mondays, Thursdays, and Fridays preferred 12-hour shifts.
