Job Summary
Provide compassionate personal care in patients' homes, assist with ADLs, safe transfers and mobility, medication reminders per care plan, monitor and report changes to supervising nurse, perform light housekeeping related to patient safety, document care in HCHB, communicate with patients and care team, follow infection control practices, assist with range of motion and restorative activities, and coordinate with supervising RN/LVN for care plan updates; field-based, full-time role in Texas with mileage reimbursement and competitive hourly pay ($17-$18) depending on experience.
Required Qualifications
- Current Texas HHA certification
- 1+ years of experience providing personal care (home health, assisted living, nursing home, or hospital setting)
- CPR/BLS certification
- Current Texas driver's license and reliable transportation
- Ability to lift, transfer, and provide hands-on care safely
- Strong communication skills and professional demeanor
- Comfortable working independently in patients' homes
- High school diploma or GED
