Job Summary
Provide personal care in patients' homes (bathing, dressing, grooming, toileting) and assist with daily living activities, transfers and mobility, medication reminders per care plan, and light housekeeping. Monitor and report changes in patient condition and vital signs to the supervising nurse; document care in Homecare Homebase (HCHB); coordinate with supervising RN/LVN for care plan updates; ensure infection control and patient privacy; support restorative care activities such as range of motion; communicate clearly with patients, families, and care team to maintain continuity of care.
Required Qualifications
- Current Texas HHA certification
- 1+ years of experience providing personal care (home health, assisted living, nursing home, or hospital setting)
- CPR/BLS certification
- Current Texas driver's license and reliable transportation
- Ability to lift, transfer, and provide hands-on care safely
- Strong communication skills and professional demeanor
- Comfortable working independently in patients' homes
- High school diploma or GED
