Job Summary
Provide personal care in patients' homes including bathing, dressing, grooming, and toileting; assist with transfers, ambulation, and mobility using proper body mechanics; support medication reminders per care plan; monitor and report changes in patient condition and vital signs; perform light housekeeping related to patient safety; document care in Homecare Homebase (HCHB); communicate with patients, families, and care team to ensure continuity of care; follow infection control practices and respect privacy; assist with range of motion and restorative care; coordinate with supervising RN/LVN for care plan updates; field-based role in Fort Worth, TX with mileage reimbursement and hourly pay between $17-$18 depending on experience; requires Texas HHA certification, CPR/BLS, and 1+ year of relevant experience; bilingual (English/Spanish) a plus; 401(k) benefits and other insurance options; flexible scheduling.
Required Qualifications
- Current Texas HHA certification
- 1+ years of experience providing personal care (home health, assisted living, nursing home, or hospital setting)
- CPR/BLS certification
- Current Texas driver's license and reliable transportation
- Ability to lift, transfer, and provide hands-on care safely
- Strong communication skills and professional demeanor
- Comfortable working independently in patients' homes
