Job Summary
Provide in-home personal care services to clients across Adams and Highland counties under the direction of a Registered Nurse or Therapist. Responsibilities include range of motion exercises, assistance with ambulation, bathing, dressing, meal preparation, light housekeeping, and medication-related support while documenting care and coordinating with the interdisciplinary team. Requires formal certification training and CPR/First Aid, valid driver’s license, and a willingness to travel up to the specified radius. Available as part-time or full-time with weekday/weekend and hybrid scheduling; starting pay is $15/hour. Report to the RN Case Manager/Administrator/Director of Nursing.
Required Qualifications
- Successful completion of a formal certification training program and/or a written skills test and competency evaluation
- CHHA/CNA/STNA accreditation desirable but not required
- Active CPR and First Aid Certifications required
- Be at least eighteen (18) years of age
- Demonstrated ability to read, write, and follow a written Plan of Care
- Current driver’s license, good driving record, and/or reliable transportation
- 1+ years of experience in home health care is highly preferred
