Home Health Aide - HHA/CNA

Provide one-on-one in-home patient care following a personalized care plan. Assist patients with daily activities such as moving in and out of beds, baths, or wheelchairs; care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care; maintain records of patient care, condition, progress, or problems to report to the supervisor; build relationships with patients through conversation or reading aloud; administer medications and immunizations; engage patients in exercises or other activities; demonstrate strong verbal and written communication skills and prior Home Health or Hospice experience is a plus.
