Job Summary
Provide Home Health Aide and Certified Nursing Assistant services under Clinical Care Manager or Registered Nurse direction. Perform personal comfort maintenance and activities of daily living for medically fragile children with acute or chronic illnesses. Execute household services and report client status changes to the interdisciplinary team. Communicate via verbal reports, staff meetings, and team conferences as requested. May perform simple procedures extending therapy or nursing services. Maintain certification through formal training, skills testing, and state registry standing. Submit clinical information and adhere to agency evaluation requirements.
Required Qualifications
- Successful completion of a formal certification training program and a written skills test and competency
- Agency evaluation as required by state and federal requirements
- No continuous period of 24 consecutive months during which none of the certified services were furnished
- Currently listed in good standing on the state Certified Nurse Aide or Home Health Aide registry
- At least eighteen (18) years of age
- CPR certified
- Complete a background check in accordance with company policy and applicable laws
Desired Qualifications
- Minimum of six (6) months' work experience in a supervised setting, preferably health care facility
