Home Health Aide - HHA

Job Summary
Assist clients with Activities of Daily Living including bathing, dressing, grooming, toileting, and meal preparation while reporting daily activity through the ClearCare management system. Provide medication assistance, housekeeping, laundry, and transportation to appointments, maintaining strict confidentiality and HIPAA compliance. Practice safety in all activities, remain vigilant of client condition changes, and document any unusual behavior or events immediately. Report suspected abuse or neglect to the proper chain of command and attend requested meetings and in-services. Maintain a valid driver's license, reliable transportation, and "Fit for Duty" physical condition while adhering to company policies and dress codes.
Required Qualifications
- Good communication skills
- Ability to follow verbal and written assignments
- Proficiency with Homecare Company Management System--ClearCare
- Maintain an established work schedule
- Effective interpersonal and communications skills including tact and diplomacy
- Effective organizational and planning skills with attention to detail and follow through
- Establish and maintain effective working relationships
- Maintain confidentiality of work-related information and materials
- Maintain 'Fit for Duty' physical condition
- Valid driver's license
- Automobile insurance
- Reliable Transportation
- 18 Years of Age or Older
- Must possess required skills and be able to explain and demonstrate, with or without reasonable accommodations, that the essential functions of the job can be performed
- Be required to lift or push up to 50 pounds
- Demonstrate honesty, accountability and integrity
- Provide superior care to Avenues Home Care clients
- Remain vigilant of any potential care concerns or change in condition
- Keep your personal appearance in compliance with Avenues Home Care Policies, including abiding by company dress code
- Be timely in reporting for shifts or making scheduling changes
- Report any suspected abuse or neglect immediately up the proper chain of command, as required by law
- Document any behavior, events, conditions or people that are out of the ordinary
- Attend meetings and in-services as requested
- Abide by all policies and procedures set forth by the Company
- Ensure protection of confidential information that is stored, handled, or maintained by following Company policies and procedures related to confidentiality and/or HIPAA regulations
Desired Qualifications
- CNA License or alternate experience as a caregiver
- CPR & First Aid Certifications
- Prior experience assisting with ADLs or caring for elderly, disabled and other vulnerable clients
