Job Summary
Monitor participant ADL/IADL care plans, document care on flow sheets, and check voicemail every two hours to report abnormal findings or status changes to the Nurse On Call. Assist participants with toileting and other ADLs per the care plan, accompany them to appointments, and perform simple treatments like catheter care and skin assessments under nursing direction. Work independently in a minimally supervised setting while respecting participants' homes and communities. Requires current H.H.A./C.N.A. certification, one year experience with frail or elderly populations, and a vehicle for travel between homes.
Required Qualifications
- Current certification as a H.H.A. /C.N.A. from an approved program.
- Must demonstrate competencies before providing participant care.
- A minimum of one year experience working with a frail or elderly population.
- Prior experience as health aide, personal care assistant or similar role.
- Vehicle to travel from home to home required
- Physical capability to provide ADL and mobility assistance to all PACE participants.
