Job Summary
Provide essential hands-on home health care to patients in their homes, including assistance with bathing, grooming, hygiene, and activities of daily living, while following the established plan of care and maintaining accurate documentation. Receive clinical support from supervising RNs and leadership, with PRN/flexible scheduling and mileage reimbursement. Role emphasizes independent field work with ongoing onboarding, training, and opportunities for advancement within Concierge Home Care. Requires transporting to patient homes across assigned routes, strong communication, and adherence to regulatory standards.
Required Qualifications
- Completion of a Home Health Aide training program meeting Medicare Conditions of Participation
- Valid driver’s license, reliable transportation, and auto insurance
- Ability to travel to patient homes throughout the day
- Strong communication and interpersonal skills
- CNA certification (preferred)
- Experience working with diverse patient populations (preferred)
