Job Summary
Provide direct assistance with activities of daily living, including personal hygiene, grooming, showering, and dressing. Support residents with environmental hygiene tasks such as laundry, light housekeeping, and maintain clean, safe living spaces. Assist with meal preparation, grocery shopping, and basic nutrition support while observing and reporting changes in residents' physical condition or mental health status. Communicate respectfully with residents and interdisciplinary team members to encourage independence and autonomy. Maintain accurate documentation in electronic health records and attend weekly supervision. Follow all agency policies regarding confidentiality and safety. Remain current with required compliance documents and trainings. Work Monday through Friday or Tuesday through Saturday or Sunday from 9am to 5pm. Willingness to travel between East Patchogue and West Sayville is required.
Required Qualifications
- High School Diploma or Equivalent
- Home Health Aide Certification from a NYS Department of Health approved training program
- Two years of experience working with adults with serious mental illness, chronic medical conditions, aging populations, or a combination of these
- Strong skills in collaboration and communication
- Willingness and ability to travel between East Patchogue and West Sayville
