Home Health Aide

Home Health Aide provides personal care and basic nursing tasks under supervision of an RN or supervising clinician, following a written plan of care. Responsibilities include taking and recording vital signs, dressing changes, bath and hygiene assistance, mobility and transfers, medication assistance as directed, light housekeeping to support patient recovery, reporting changes to the supervising clinician, and coordinating with the interdisciplinary team. Requires ability to work in patient homes and meet California CNA/vital-signs requirements; certification/licensure is required. Schedule is Mon-Fri 8:30am-5pm with rotating weekends; location includes Torrance, CA. The role involves collaboration with RNs, Physical/Occupational Therapists, and Speech Language Pathologists to implement and adjust the plan of care; training and documentation are ongoing, with emphasis on patient dignity, safety, and independence, and ability to work in pet-friendly home environments.
