Home Health Aide

Home Health Aide in a home-health setting provides personal care and basic nursing tasks as outlined in the patient’s plan of care, under supervision of an RN Clinical Case Manager (or a supervising Physical Therapist/Occupational Therapist/Speech Therapist if no RN on the case). Responsibilities include bath and hygiene assistance, dressing, skin care, mobility and transfers, meal/ambulation support, medication assistance as directed, vital signs, simple dressing changes, observing changes in condition, and recording patient status. The role works within the care team to develop and update the plan of care, completes documentation, coordinates with scheduling and care staff, and participates in required in-service education to maintain clinical skills. The position requires CNA licensure (CA), BLS certification within 30 days of hire, and compliance with agency policies, including safety and infection control standards. The role may involve exposure to pets in the home and requires collaboration with the interdisciplinary team to support patient independence and health at discharge.
