Job Summary
The Home Health Aide provides direct in-home care per the patient’s plan of care under RN or therapist supervision. Responsibilities include personal care (bath, oral/denture care, skin/hair/nail care), assistance with daily living activities, basic nursing tasks (vital signs, dressing changes, catheter/prosthetic device care as instructed), medication assistance for self-administering patients, light housekeeping to support recovery, and documentation of patient status. You will work within a team to implement and update the plan of care, coordinate with other clinicians, and participate in required trainings. CA CNA licensure and BLS are required; the role emphasizes compassionate, confidential care, patient safety, and collaboration with the care team in home-based settings.
Required Qualifications
- Certified Nursing Assistant (CNA) license issued by the California Department of Health
- BLS certification within 30 days of hire (American Heart Association)
