Home Health Aide

Assist patients with daily activities including moving in and out of beds, baths, wheelchairs, or automobiles. Provide personalized care by changing bed linens, doing laundry, cleaning the home, and administering medications and immunizations. Maintain detailed records of patient condition and progress to report observations to the supervisor. Build relationships with patients through conversation or reading aloud to support mental health and alertness. Engage patients in exercises and other activities tailored to their needs. This role requires a one-on-one care plan approach in patient homes, with a focus on creating a healthy environment.
