Job Summary
Provide routine nursing care and assist with resident admissions, transfers, and discharges in accordance with individualized care plans. Support activities of daily living, observe and document changes in resident conditions, and complete required documentation in electronic medical records. Assist with meal service, maintain a clean and sanitary environment, and safely assist residents with mobility and transfers up to 50 pounds. Observe, document, and communicate changes in residents' condition to the nursing team while participating in training, in-services, and team meetings. This onsite role in Ponte Vedra Beach, FL, supports residents' physical, emotional, and social well-being under nursing leadership. Active CNA and CPR certification are required, with opportunities to expand clinical skills and explore advancement within Health Services and Home Care.
Required Qualifications
- Active Certified Nursing Assistant (CNA) certification
- Active CPR Certification
- Compassionate and service-oriented approach to resident care
- Strong interpersonal and communication skills
- Ability to build trusting relationships with residents, families, and team members
- Ability to accurately document care and follow established procedures
- Strong observation skills and attention to detail
- Ability to work independently and manage priorities effectively
- Ability to safely assist residents with transfers, mobility, and activities of daily living
- Ability to lift, transfer, push, or support residents as required, including lifting up to 50 pounds
- Ability to maintain resident confidentiality and professional standards
- Valid driver's license and acceptable driving record if transportation duties are assigned
- High school diploma or GED
- Must be available for weekend shifts
- Must be able to lift 50 lbs
Desired Qualifications
- Previous experience in home health, skilled nursing, assisted living, memory care, or senior living
- Experience with electronic medical record (EMR) systems
- Knowledge of dementia care and aging-related health conditions
- Experience providing transportation assistance and companionship services
- Home Health Aide experience
