Job Summary
Assist clients with daily living tasks as outlined in a care plan developed by the RN or Supervisor of Clinical Services. Perform personal grooming and hygiene, assist with mobility and transfers, provide nutrition support, and offer toileting and elimination assistance. Retrieve medication containers, confirm prescriptions, remind clients to take medication, and observe self-administration. Conduct light housekeeping necessary for client safety. Assigned to clients by an RN with care instructions prepared and updated as needed. Complete agency orientation and pass documented competency assessments by a qualified RN.
Required Qualifications
- At least 18 years old
- Minimum one year of verifiable personal care experience
- Completion of HHA training or CNA certification
- Competency evaluation
- Physically able to perform duties (lifting, transfers, ambulation)
- Must read, write, and speak English fluently
- Reliable transportation to and from assignments
- 3 references
- Must complete agency orientation
- Pass documented competency assessments by a qualified RN
