Job Summary
Provide compassionate Home Health Aide/Hospice care as part of an interdisciplinary team under supervision of a registered nurse or therapist. Deliver personal care and psychosocial support including bathing, grooming, toileting, mobility assistance, and adherence to Care Plan. Assist with meals, medications prompting, light homemaking, and transportation to medical appointments. Utilize transfer devices (e.g., Hoyer lift), support prescribed exercises, and ensure infection-control best practices. Requires CNA license and Virginia driver’s license; hospice/home-health experience preferred.
Required Qualifications
- Valid unencumbered Certified Nurse Assistant license
- Prior Hospice or Home Health Experience is preferred
- Valid Virginia Driver’s License with a good driving record
- Influenza vaccine requirement (and COVID-19 vaccines recommended)
