Job Summary
Provide direct patient care under RN direction and according to the Plan of Care (POC). Assist with self-administered medications, monitor vital signs, and report abnormalities. Support personal hygiene, feeding, nutrition, mobility, transfers, and daily living activities. Maintain infection control, document care interactions with patients and caregivers, and ensure a safe home environment. Requires driving capability and compliance with TB screening and driver-safety requirements. Schedule is 40 hours per week with competitive benefits.
Required Qualifications
- Complete Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months
- Must meet applicable state certification requirements
- A valid driver's license, auto insurance, and reliable transportation are required
- Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare certified agency
- Minimum one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
