Job Summary
Provide direct physical care and basic care procedures for stable patients, including mobility, transfers, repositioning, and assistance with activities of daily living. Apply wound dressings, perform specialized skin care, and manage tube site care for NG, G, or J tubes. Collect and label urine or stool specimens, perform catheterization, and assist patients with therapy-prescribed home exercise programs. Maintain equipment and supplies under clinician direction while communicating pertinent observations and documenting changes in the Electronic Health Record. Report unexpected events or condition changes to the RN Case Manager promptly. Maintain patient privacy and confidentiality throughout all interactions. Work schedules are determined by patient care needs and may include weekends and holidays for both scheduled and per-diem roles. Minimum scheduled expectation is 0.5 FTE, with per-diem roles requiring at least one shift monthly. State of Wisconsin Professional licensure for Certified Nursing Assistant and CPR/BLS certification are required.
Required Qualifications
- State of Wisconsin Driver's License
- State of Wisconsin Professional licensure for Certified Nursing Assistant
- High School Diploma or equivalent
- A minimum of one (1) year of recent experience as a Nursing Assistant
- CPR/BLS certification
- Valid Auto Insurance
- Reliable vehicle transportation
- Work schedules determined to meet patient care needs
- Work may be required on weekends and holidays
- Minimum scheduled role is 0.5 FTE
- Minimum work expectation for a per-diem role is one (1) shift/month
