Job Summary
Provide in-home care to older adults as part of the LifeCircles PACE team, assisting with personal care and activities of daily living in the participant's home. Support housekeeping tasks, mealtime, mobility and safe transfers, monitor vital signs, and report changes to the interdisciplinary team. Maintain confidentiality and deliver compassionate, respectful care while following each participant's Life Plan. Requires certification as HHA or CNA and a high school diploma or GED; reliable transportation and strong communication/observation skills are essential.
