Job Summary
Provide direct patient care under the direction of a registered nurse, assisting with safe transfers, ambulation, nutrition, and fluid intake while maintaining infection control standards. Document pertinent care rendered and report changes to ensure continuity, then participate in weekly case conferences and interdisciplinary communication to optimize service delivery. Promote positive communication with patients, families, and the clinical team while ensuring patient safety and privacy. Maintain competency through annual in-services and contribute to efficient agency operations.
Required Qualifications
- Completion of Certified Home Health Aide Program
- 1 year of Home Health Aide experience in a home health agency
- Reliable transportation
- Valid and current auto liability insurance
- Good communication skills to be able to relate professionally and positively to patients, caregivers, and clinical team
- Ability to read, write, and speak the English language
- Demonstrates interest in the welfare of ill and elderly
- Proof of current, CPR and Hepatitis profile
- Aide Competency Evaluation Skills Checklist and Written Skills Test
- Demonstrate commitment, professional growth and competency by attending 12 hours of in-services per year
