Job Summary
Assist clients with Activities of Daily Living including bathing, dressing, oral care, toileting, transfers, and turning to prevent pressure injuries. Provide meal planning, preparation, feeding support, catheter/colostomy care, and light housekeeping while accompanying clients to appointments. Engage clients in recreational activities and offer emotional support, then accurately document all care, vital signs, and intake/output on the same day. Promptly report any changes in client condition or hospitalization to the RN Case Manager, maintaining infection control and confidentiality while strictly adhering to the Plan of Care.
Required Qualifications
- High school diploma or GED
- Successful completion of an approved HHA training program
- Completion of required competency testing
- Current CPR/First Aid certification
- TB screening within the past year
- Clear state background check
- Reliable transportation with valid auto insurance
- Ability to lift at least 25 pounds
- Ability to read, write, and communicate effectively in English
- Strong communication skills and ability to follow written and verbal instructions
