Conduct admission assessments, care planning, goal setting, and evaluation for patients within a 60-mile radius of the Otsego office. Complete OASIS assessments at prescribed intervals, administer medications per physician orders, and monitor patient status while maintaining clinical documentation. Collaborate with clients, families, physicians, and other disciplines to coordinate care and identify potential referrals. Provide instruction to patients and families regarding illness, treatment regimens, and health maintenance. Work Monday through Friday with weekend rotation, earning $90,000–$105,000 annually with mileage reimbursement and a company vehicle option.
