Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders for patients in the Lancaster, PA area. Educate and guide patients and families through the admission process, setting expectations for care and ensuring they feel informed from day one. Apply strong clinical judgment to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation while traveling to patient homes within your assigned territory. Work independently with support from a dedicated field education team. Requires one year of RN experience, an active license, and a current driver's license.
