Job Summary
Home Health Admissions RN responsible for leading and collaborating to manage and deliver patient care plans in Harrisburg, PA and surrounding areas. Requires RN license in the state of work, OASIS experience, and prior home health nursing experience. Must hold a valid driver’s license and be able to drive ~20% of the day. Proficiency with EMR software and mobile/tablet devices is required. 40 hours per week, full-time with a sign-on bonus and a comprehensive benefits package; role involves coordinating with clinical educators/supervisors and utilizing a patient-care-team approach to ensure timely charting and delivery of care plans.
Required Qualifications
- RN license in the state you work
- Graduate from an approved school of practical nursing
- Previous Home Health nursing experience
- Previous OASIS experience
- One year of RN experience
- Current driver’s license
- Ability to spend ~20% of your day driving
- Experience with tablets, mobile phones and EMR software
