Home Dialysis Patient Care Technician - CCHT

Guide new patients through a 120-day education plan, obtain and record vital signs and machine readings during treatment, and set up and break down dialysis blood systems. Insert and remove patient access needles while monitoring patients, documenting changes, and informing the charge nurse of concerns. Calculate weight loss to reach dry weight, determine care priorities, and organize workloads. Maintain professional relationships, observe privacy rights, track inventory, and perform laboratory work under RN supervision. This role supports the interdisciplinary team in ensuring safe care for end-stage renal disease patients within our nonprofit dialysis program.
