Job Summary

Monitor services, create new strategies to reach goals, and coordinate services to empower individuals to meet their objectives. Build and maintain team culture through active presence, monthly meetings, staff coaching, and on-call participation for short-notice staffing gaps. Manage operational resources including vehicle usage, individual budgets, payee processes, medication check-ins, and appointment scheduling. Ensure person-centered service delivery by collaborating with case managers and interdisciplinary teams, developing individualized plans, and maintaining accurate documentation and authorizations. Serve as the primary liaison for individuals, families, and guardians while advocating for independence and compliance with all applicable laws and regulations. This full-time role operates Monday through Friday, 8:00 a.m. to 4:30 p.m., with participation in an on-call rotation and flexibility for additional coverage.

Required Qualifications

  • High School Diploma/GED equivalent
  • Must be able to work in the U.S. without sponsorship
  • At least 18 years of age
  • Valid driver's license
  • Reliable transportation
  • Must be able to drive and transport for service provision as needed
  • Must wear hearing and eye protection in required areas
  • Must have ability to see objects/persons at a distance
  • Must have ability to see close work such as typed or handwritten material
  • Must have ability to hear conversations in a quiet and a noisy environment
  • Must have ability to determine where a sound is coming from
  • Must have ability to communicate through speech
  • Must remain awake during working hours, if applicable
  • Must be able to lift up to 35 pounds
  • Must be able to use a two-person lift, mechanical lift, or assistive equipment for lifting more than 35 pounds
  • Must be able to communicate effectively to the rest of the team and the people we serve
  • Must be self-aware enough to know when to stand up as an advocate for others and when it's time to follow the leader
