Job Summary
Coordinate home assistance for Winnebago public health nursing patients and community members, including bathing, vitals checks, and transfer support as directed by the senior citizen's center director. Prepare and document visit reports for follow-up with nursing staff while reporting any hazards to patients. Maintain strict confidentiality regarding sensitive information and assist with meal deliveries when needed. Complete all work order assignments timely and adhere to personnel policies. This role operates on a PRN basis and requires the ability to lift over 50 lbs, walk, and stand for extended periods. Applicants must possess a valid driver's license, be insurable under the Tribe, and demonstrate strong time management and customer service skills.
Required Qualifications
- High school diploma or GED
- Nursing assistant certificate
- Must be polite and respectful to all relatives
- Must possess above average time management skills
- Must be sensitive to the requirements of the patients
- Must possess above average customer service skills
- Must be punctual and dependable
- Knowledge of cleaning procedures and practices
- Ability to read and follow instructions
- Knowledge of general safety practices
- Knowledge of medical equipment or ability to learn
- Ability to meet physical requirements, such as lifting, bending, and standing for duration of shift
- Must have a valid Driver's License and be insurable under the Tribe
- Must possess the ability to read, write, and speak the English language fluently
- Must be able to employ professional verbal and written communications skills continually and effectively
- Must possess above average social interaction skills
- Works with minimal to no supervision
- Manual dexterity with cooperatively free range of motion of all extremities including neck, fingers, hands, wrists, elbows, arms, shoulders, waist, legs, hips, knee joints, ankles, and feet
- Ability to lift over 50 lbs
- Regularly required to walk, stand, and sit for extended periods of time

- Must comply with 45 CFR 1301, Subpart D, Section 1301.31, C and D regarding child abuse and violent felony declarations
