Job Summary
Conduct patient assessments and develop individualized transition plans to facilitate safe discharge from skilled nursing facilities. Coordinate referrals and build relationships with physicians, discharge planners, and healthcare partners to ensure seamless care transitions. Serve as a key ambassador for Catholic Home Care within the community while identifying opportunities for program growth and service expansion. Collaborate with interdisciplinary teams to deliver exceptional support to patients, families, and partners. This role requires a Registered Nurse with at least two years of home care experience, including patient assessments, and involves rotating weekends and holidays.
Required Qualifications
- Current Registered Nurse (RN) license in New York State
- Minimum of 2 years of home care experience, including patient assessments
- Strong relationship-building, communication, and customer service skills
