Job Summary
Conduct patient assessments and develop individualized transition plans to support safe discharges. Coordinate referrals and facilitate smooth transitions to home care services while building relationships with physicians, discharge planners, and referral sources. Serve as a key ambassador for Catholic Home Care within the community, identifying opportunities for program growth and service expansion. Collaborate with interdisciplinary teams to ensure positive patient outcomes and deliver exceptional customer service to patients, families, and healthcare partners. Work rotating weekends and holidays in our Farmingdale office.
Required Qualifications
- Current Registered Nurse (RN) license in New York State
- Minimum of 2 years of home care experience, including patient assessments
- Strong relationship-building, communication, and customer service skills
