Home Care Supervisor / RN - PRN - Bilingual Korean/English

Make initial contact with patients, complete start-of-care assessments, and design individualized plans of care for clients in the Annadale VA area. Instruct assigned care team members on care plan details, report progress to involved parties, and maintain relationships with clients while advocating on their behalf. Establish relationships with community partners, review caregiver documentation, and participate in interdisciplinary team meetings to ensure patient needs are met. Keep Clinical Leadership and Office Administrator abreast of patient conditions and medical changes. Requires active Virginia RN license, Associate's degree, and bilingual Korean/English skills.
