Job Summary
Coordinate care and manage schedules for caregivers and patients in private duty home care settings across Santa Fe, Española, and surrounding areas. Ensure services are delivered efficiently by working closely with families, healthcare professionals, and caregivers to develop personalized care plans and maintain adequate shift coverage. Solve problems quickly regarding caregiver availability, patient needs, or scheduling conflicts. Assist with admission documentation for new patients and support hiring and training of new caregivers. Perform duties as a caregiver and Home Health aide during emergencies. Work after-hour shifts and weekends as needed. Must have reliable transportation and ability to perform physical tasks including lifting 50 lbs.
Required Qualifications
- High school diploma or equivalent
- Must be able to work after hour shifts and weekends as needed
- Must have reliable transportation
- Ability to perform physical tasks, including lifting 50 lbs or assisting with mobility
- Dependable, punctual, and trustworthy
Desired Qualifications
- Previous caregiving or healthcare experience
- Strong communication skills and a compassionate, patient approach
- Basic understanding of health and safety guidelines
- CPR and First Aid certification (or willingness to obtain)
