Job Summary
Provides safe, holistic care to home-bound patients with multiple co-morbidities and advanced chronic illnesses; educates patients and families on self-management; travels within the service area to deliver home care; collaborates with team to support quality, compassionate, patient-centered care; requires med-surg and home care experience, IV/venipuncture skills, CPR, and ability to work weekends/holidays/on-call; must have current licensure in practice state and valid driver’s license; proficient in using technology (cell phone, computer).
Required Qualifications
- Minimum of 1 year med-surg experience required
- 1 year of home care experience preferred
- BSN preferred
- able to work weekend, holiday, and on-call hours
- IV and venipuncture experience preferred
- able to demonstrate professional competencies in a home care setting
- able to efficiently utilize technology (cell phone, computer)
- able to travel in service area
- Current licensure in State of practice
- Valid Driver's License
- Current CPR Certification
