Job Summary
RN plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education. Develops and revises care plans, administers medications and treatments as prescribed, and provides direct patient care per the State Nurse Practice Act. Coordinates patient care as Case Manager when assigned, conducts initial and ongoing assessments, and counsels patients and families. Responsible for discharge planning, communicating with physicians, and supervising home health aides. PRN day shift with weekend availability and on-call duties; requires state licensure, CPR certification, and a valid driver’s license. Located in Durham, NC; comprehensive benefit package and PTO included.
Required Qualifications
- Graduate of an accredited school of nursing
- Current licensure in state as RN
- CPR certification
- Valid driver’s license
- Bachelor’s or Associates degree in Nursing (home health care experience preferred)
- One to two years of recent experience in a healthcare setting
- Ability to supervise home health aides
- Excellent observation, verbal and written communication skills
- Prolonged walking/standing and ability to lift/move patients and equipment
