Job Summary
Delivers skilled nursing services and performs comprehensive physical assessments for clients and families in home and community settings. Collaborates with physicians and multidisciplinary teams to develop, evaluate, and update coordinated plans of care, ensuring accurate communication of physician orders. Initiates case conferences to discuss client progress and care adjustments while maintaining a safe environment for all parties. Requires a Registered Nurse license, Associate's degree, and one year of post-acute care experience. Full-time role with 64 hours bi-weekly, weekdays 8am-4:30pm, plus 9th weekend and holidays. Local travel required.
Required Qualifications
- Associate's Degree in Nursing
- 1 year of nursing experience in post-acute care setting
- Must be licensed as a Registered Nurse within the state in which working and must maintain licensure
- Ability to communicate effectively both verbal and written
- Proficient with Microsoft Office Suite
- Ability to follow all safety rules, regulations, policies and procedures of the facility including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precaution, AWAIR, OSHA, Right to know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations
