Job Summary
Provide prescribed medical treatment and personal care to terminally ill patients in collaboration with a transdisciplinary team across settings. Coordinate the Plan of Care using the Nursing Process; assess physical, psychosocial, and spiritual needs; monitor symptoms and pain; serve as liaison to physicians; offer emotional support to patients and families; educate patients and families; complete plans and reports related to hospice care; participate in care conferences, quality improvement activities, on-call rotation, discharge planning, and coordination with volunteers. Maintain precaution protocols and participate in education/precepting of staff and students; flexible hours to meet patient/family needs; engage in community outreach and maintain a compassionate, patient-centered approach according to agency standards.
Required Qualifications
- Licensed RN in State of Ohio
- Current CPR certification
- Must have and maintain a valid driver’s license
- Ability to provide proof of eligibility to work in the United States
