Per Diem/PRN RN in home care coordinating Start of Care, Re-certification, Discharge, and Resumption of care. Provides direct patient care including medications, diagnostic tests, and vitals; collaborates with physicians to develop patient care plans; educates patients/families about conditions, treatment options, and preventive care; assists with discharge planning and post-discharge care; delivers specialized dementia care.
Required Qualifications
- Valid Registered Nurse (RN) Required in the state of Texas
- Minimum 2 years experience in home care experience
- Effective written and verbal communication
- Experience with policy, rules, and regulations
- Clean background check and driving record
