Job Summary
Provides patient care services under the supervision of a Physical Therapist, updates and coordinates the care plan with the therapist and physician, plans monthly re-evaluations and schedules, documents patient data showing progress, and communicates with the patient/family and care team. Coordinates referrals and supervisory visits, participates in meetings and in-services, and ensures ongoing documentation and performance of duties. Requires completion of an accredited physical therapy assistant program and current state licensure, CPR certification, and ongoing continuing education; travel and variable working conditions are expected. EOE.
Required Qualifications
- Requires completion of an accredited physical therapy assistant program
- current State licensure
- Minimum of one year’s therapeutic experience in a non-home health care environment; prefer two years
- Strong knowledge of theory and concepts relating to the practice of physical therapy
- CPR certified and possess strong communication and organization skills
- Minimum of twelve hours of continuing education annually
- Must accept travel and working conditions as assigned
